The STOP HIV/AIDS Outreach team provides HIV, STI and Hepatitis testing via outreach clinics in community settings. The team also provides clinical case management and primary care support for patients living with HIV who are newly diagnosed and/or having difficulty engaging in care, or who are lost to follow-up. The team consists of Nurses, Social Workers, Outreach Workers and peers who work in a team to provide the following services: Outreach, linkage to care, blood work, immunizations, HIV medication adherence supports, and care coordination including assistance with housing and income stability, food security, and social support. The phone line is open to health care providers 7 days a week between 7am and 10pm for referrals or advice.

To engage clients who experience barriers to HIV testing and care and to improve system coordination for persons living with HIV.
