The Diabetes Education Centre has a multi-disciplinary team of health care professionals dedicated to provide education for prevention and self-management of diabetes for healthy living.

We support clients of all ages and backgrounds and can arrange for interpreter services for non-English speaking clients. Interpreters for all clients are provided by VCH as required, usually by telephone.

Group education for type 2 diabetes and pre-diabetes is offered. Individual appointments are determined by the Team for clients not suitable for group education. Also offered: Insulin Therapy; Diabetes in Pregnancy; Pediatric Diabetes Clinic; Young Adult Type 1 Diabetes Clinic (for clients transitioning to adult care); Diabetes Foot Care. Some fees may apply.

To support to GPs in their practice, we can also offer: Medical Group Visits and Mobile Clinics.

Mandate

To assist individuals self-manage their condition by providing assessment, education and treatment.

Service Hours

Reception: Monday to Friday; 8:00 am - 4:30 pm.

Program Team:

Diabetes Nurse Clinicians, Clinical Dietitians, Exercise Specialist. Endocrinologists consulted as required.
